The GoA Vaccination Exemption Group sent out a new form to many MyAPSChoice Group members who indicated previously that they were not prepared to complete the vaccination profile in 1GX. The new form has removed the requirement for a signature by a Commissioner of Oaths, but it still contains “fine print” that would solicit consent from you for items that are inappropriate.
The MyAPSChoice Group recommends you DO NOT use, sign or submit that form.
Rather, complete the modified template in limited fashion. The template includes several end-notes that will secure your greater protection. You then can include your general exemption request (labelled APPENDIX A) as previously recommended in related these posts:
When completing the MyAPSChoice exception template, print it, hand-write only your personal particulars, date and sign the form, then scan it for inclusion with your general exemption request which you should label as “APPENDIX A”.
