MyAPSChoice Class Action Instructions

The MyAPSChoice Legal Counsel, Mr. Jeff Rath, is now beginning to prepare the group legal action on behalf of employees of the GoA. If you wish to become part of any future class action or group action, then please following these instructions:

1. Go to this webpage: https://rathcocovidlitigation.com/
2. Select “Send Information”
3. Fill-in all of the fields in the form.
4. In the “Message” section briefly answer / identify these circumstances:
   1. You are a member of the MyAPSChoice Group.
   2. You are providing your consent to be named in a MyAPSChoice group legal action.
   3. You are prepared to complete a legal Affidavit to support a Statement of Claim.
   4. You were either denied a human rights exemption, forced to be tested twice per week, coerced to consent to be vaccinated, have been placed on a leave of absence, or all.
   5. What you have and have not consented to as part of the GoA Vaccination Policy (ie: 1GX form, exemption submission etc).
   6. Whether or not you submitted a Notice of Liability to anyone.
   7. Whether or not you filed a grievance with AUPE.
   8. The results or status of your grievance.
   9. If you filed any personal complaints to (for example) the Human Rights Commission besides what the MyAPSChoice Group prepared.
   10. If you have suffered financial loss because of the GoA Vaccination Policy.
   11. If you have suffered medical or mental harm (no details) because of the GoA Vaccination Policy.
